Terms: The requesting company must be a current Cambridge Integrated Services Group, Inc. client. Borrowed video(s) must be returned on or before the 14th day they are due in the Risk Control office located at 175 West Jackson Blvd, Suite 1000, Chicago, IL 60604. Two (2) videos maximum per request, additional videos can be requested after the previous order has been safely returned. The client pays for the return shipping of the videos. All videos must be returned via a shipping service that can provide tracking and proof of delivery. Videos should be insured for $250.00 each to cover the replacement cost in the event they are lost or stolen during return shipping. Videos that are lost, damaged or stolen while in client control are the responsibility of the client and will be replaced, through our vendor at the actual current replacement cost.
